If I cannot provide consent or the minor’s parent or legal guardian cannot be reached promptly, I authorize emergency responders and licensed medical professionals to provide evaluation, transportation, and treatment as permitted by law. I accept responsibility for costs for which I am legally responsible.

SECTION 4 — WASHINGTON STATE EQUINE ACTIVITY STATUTE

Notice required by Washington State law (RCW 4.24.540): Under Washington law, an equine professional is not liable for an injury to, or the death of, a participant in equine activities resulting from the inherent risks of equine activities. By signing this document, you acknowledge that you are aware of this statutory provision and are voluntarily assuming the risks associated with equine activities.

SECTION 5 — ACKNOWLEDGEMENT OF INHERENT RISKS

I understand and acknowledge that equine activities — including but not limited to riding, ground work, lunging, in-hand work, handling, and observing horses at close range — involve significant and inherent risks. These risks exist regardless of the skill level of the participant or the care taken by organizers, instructors, or facility staff. I acknowledge that the following risks are inherent to equine activities and cannot be eliminated:

  • Horses and other equines can be unpredictable and may react suddenly to sounds, movement, weather conditions, other animals, or unfamiliar stimuli

  • Horses may kick, bite, strike, rear, bolt, buck, or otherwise behave in ways that cause injury to riders, handlers, or bystanders

  • Falls from horses can occur even among experienced riders and may result in serious injury

  • Equipment, tack, and footing conditions may fail or contribute to accidents

  • Horse-to-horse interactions in group settings, including during clinic exercises at the instructor’s direction, may create hazards

  • Mounting, dismounting, and performing exercises at the clinic instructor’s direction carry inherent risk

  • Spectators and auditors in or near equine facilities may be exposed to risks from loose or startled horses, equipment, and facility conditions

I understand that participation in or attendance at these activities may result in personal injury, accident, illness, or death, including but not limited to sprains, broken bones, concussion, head or neck injuries, paralysis, dismemberment, or death, as well as damage to or loss of personal property. No warranty, express or implied, is made regarding the habits, disposition, suitability, training, or physical condition of any horse or animal.

SECTION 6 — RELEASE OF LIABILITY — READ CAREFULLY

In consideration of being permitted to participate in activities at or visit Seven Hills Training LLC, I, on my own behalf and on behalf of my heirs, personal representatives, and assigns, release and discharge Seven Hills Training LLC and its owners, members, managers, employees, instructors, agents, and volunteers acting within the scope of their duties (“Released Parties”) from claims for personal injury, death, or property damage arising from my participation or visit, INCLUDING CLAIMS CAUSED BY THE ORDINARY NEGLIGENCE OF A RELEASED PARTY, to the fullest extent permitted by Washington law.

This release does not apply to gross negligence, reckless or willful misconduct, intentional injury, or other liability that cannot lawfully be released.

If signing as a parent or legal guardian, I consent to the named minor’s participation and acknowledge the risks. Any release of claims I personally hold applies only to the extent permitted by Washington law. I do not purport to waive the minor’s independent claims that cannot lawfully be waived.

Washington law governs this agreement. If a provision is found unenforceable, the remaining provisions remain effective to the extent permitted by law.

SECTION 7 — FACILITY POLICIES

Safety & Conduct

  • Mounted participants must wear a properly fitted, securely fastened ASTM/SEI-certified equestrian riding helmet and appropriate riding boots. Hairstyles and accessories must not interfere with helmet fit.

  • Riders must wear clothing covering their shoulders and legs. Tank tops, spaghetti straps, skirts, capris, and shorts are not permitted for riding. Riding gloves are recommended.

  • Everyone entering horse-handling areas must wear sturdy, closed-toe footwear and dress appropriately for the weather.

  • Follow posted rules and staff instructions. Enter arenas, paddocks, pastures, stalls, and other horse activity areas only with permission. Visitors must remain in designated observation areas unless invited elsewhere.

  • Do not approach, feed, or handle a horse without permission. Avoid excessive noise, sudden movements, flashing lights, or other behavior that may startle horses. Park in designated areas.

  • Parents or guardians must supervise minors before and after instruction. Accompanying children must remain supervised at all times.

  • Pets are not permitted on the grounds. Contact Seven Hills Training in advance regarding service-animal access and safe arrangements.

  • Staff may stop an activity or ask a participant or visitor to leave if their behavior is unsafe or disruptive.

Lesson & Training Policies

These policies apply when relevant to your activities. Separately agreed written program or service terms govern where they differ.

  • Payment: Individual lessons are payable at the time of the lesson. Lesson and training packages are payable 14 days before the applicable term and must be paid in full before its first lesson.

  • Participant cancellations: Give the instructor at least 24 hours’ advance notice. Missed lessons and cancellations with less than 24 hours’ notice are not eligible for makeup time, credit, or refund.

  • Seven Hills cancellations: If Seven Hills Training cancels a lesson, a makeup lesson or credit will be arranged.

  • Weather: Activities may proceed in seasonal weather when safe. Seven Hills Training may cancel or modify activities because of weather, footing, facility conditions, or other safety concerns. When possible, we will attempt to provide at least two hours’ notice; sudden changes may require shorter notice. Monitor messages before traveling.

  • Late arrival: We will attempt to accommodate arrivals up to 15 minutes late without extending the lesson’s scheduled end time. After 15 minutes, the instructor will decide whether the lesson can safely continue or must be canceled.

  • Horse assignments: School horses are assigned at the instructor’s discretion and may change based on suitability, welfare, availability, and safety.

Keeping Information Current

Keep your contact, emergency, and safety information current and review it annually. A new signed form may be required when this agreement changes or a minor turns 18. Medical clearance may be required before returning to riding after an injury. Participants are encouraged to keep tetanus immunization current.

Discuss safety concerns, limitations, or accommodation needs privately before participating. Contact (425) 533-3738 or info@sevenhillstraining.com.

SECTION 8 — OPTIONAL PHOTO & VIDEO PERMISSION

Photo permission is optional and is not a condition of participation. Select one option.*

This choice does not grant permission on behalf of other adults or accompanying family members. Permission to publish a participant’s name or tag a personal social media account must be obtained separately.

SECTION 9 — VOLUNTARY PARTICIPATION & ASSUMPTION OF RISK

I voluntarily choose to participate in activities at or visit Seven Hills Training with knowledge of these risks. If signing for a minor, I give permission for the named minor to participate.

I will accurately describe the participant’s experience, disclose limitations relevant to safe participation, follow staff instructions, and use required protective equipment. I may decline an activity or ask to stop and will promptly tell staff if assistance is needed.

For my own participation or visit, I knowingly assume the inherent risks of these activities. If signing for a minor, I acknowledge those risks and agree to the applicable safety and supervision requirements. This acknowledgment does not waive rights that cannot lawfully be released.

SECTION 10 — ACKNOWLEDGEMENT & SIGNATURE

By signing below, I confirm that I am at least 18 years old and the information provided is accurate. If signing for a minor, I confirm that I am their parent or legal guardian and authorize their participation.

I have read and understand the emergency medical authorization, acknowledgment of risks, release of liability, and applicable facility policies. I understand that this agreement affects legal rights, including certain claims for ordinary negligence. My photo permission is governed by the separate choice above.

I agree voluntarily and intend my electronic signature to authenticate this agreement.
